Current Research

Operating Room Scheduling Efficiency: A Simulation and Optimization Approach

A three-year census of 35,772 orthopaedic surgical cases at a multi-campus academic health system, used to characterize baseline OR performance, build and validate a discrete-event simulation of the suite, identify the empirical drivers of OR-day overtime, and evaluate a CP-SAT scheduling optimizer against both observed practice and a greedy heuristic.

Three findings carry the work: delay cascades are the dominant structural driver of overtime; the optimizer eliminated scheduled overtime on a representative week and cut simulated daily overtime hours by 77%; and the institution’s block-governance rules produce a governance paradox in which the surgeons who most justify two-room access generate the most overtime when that access is withdrawn.
